Trustmark Voluntary Benefits Announces Claims Change of Address

LAKE FOREST, Ill. – Trustmark Voluntary Benefits has changed the address policyholders use to submit claims information effective immediately. Online voluntary benefits claims submission through the website portal is the fastest and easiest way for policyholders to submit a claim. If a policyholder does not have online/Internet access, the claims address can be used to provide both written notice of a claim for a covered loss and to return completed forms providing proof of loss.

If claims documentation was recently mailed to the former address, it will be processed as usual. Our forwarding service expires December 31, 2023.
