What we'll do:
- Star Marketing and Administration, Inc., will submit this data* on your behalf, but we require your input to complete the submission accurately.
What employers are required to do:
- Click on the “SUBMIT INFORMATION” button and answer the survey questions by March 21, 2023.

Here's what we need from you:
- Provide required information about your company and plan. Click on the button above to enter all information that is requested.
- Provide the required employee and employer contribution for your plan in 2022 as a percentage of total plan costs. Review the chart below to see how calculations are made.
Calculation Example for Required Contributions for Group Plans in 2022 |
Category | Required contribution for an employer-sponsored health benefit plan in 2022 | Calculation (Payments in 2022 for Category/Total payment = percentage to report) | Percentage reported |
Covered employees’ required contribution | $100,000 | $100,000 / $300,000 = 33.3% | 33% |
Employer’s required contribution | $200,000 | $200,000 / $300,000 = 66.6% | 67% |
Total | $300,000 | | 100% |
Please be sure the two percentages add up to 100%. Total payment is made to Star Marketing and Administration, Inc., by employer.
- If you filed an IRS Form 5500 for your plan, we’ll need the three-digit plan number that you reported. Review the image below to see where to find your plan number. If you did not file a Form 5500, please enter N/A.

The data will show spending on prescription drugs and healthcare services, prescription drugs that account for the most spending, drugs that are prescribed most frequently, prescription drug rebates from drug manufacturers, the required contribution for each employer-sponsored plan in 2022 for covered employees, as well as cost-sharing for covered employees.
