You Have Freedom of Provider Choice With Your Health Benefit Plan

What PPO network do I use? Under your employer-sponsored, self-funded health benefit plan, there are no networks for most covered healthcare services.1 Because there are no networks for most care under your plan, there is no list of network physicians. You have freedom of provider choice for most covered healthcare services. Without a network, how do I find a doctor? You can choose to stay with your current doctor. If you are looking for a new physician, ask for recommendations from your primary care physician, a nurse or local hospitals. Talk to family and friends or visit websites with information on doctors. A Care Navigator2 from Advanced Medical Pricing Solutions (AMPS) can help you find providers in your area, and assist with scheduling an appointment with a contracted provider for high-cost diagnostic imaging and non-emergent, elective surgical procedures. Contact Star Marketing and Administration, Inc., at 800.522.1246, ext. 26300, and ask to speak with an AMPS Care Navigator. My doctor is not familiar with my plan. What should I do? Ask your doctor to call Customer Service at 800.522.1246.Star Marketing & Administration, Inc., is the administrator of your plan. It is not a PPO network.
